《死亡之島》主要是以南太平洋某個渡假小島為背景，採用開放式世界設計；遊戲主要描述渡假小島突然遭遇不知名的病毒大量擴散，島上民眾互相感染變成殭屍。玩家將扮演倖存者，在這座「死亡之島」上找出求生之路。
此次釋出的圖片中，玩家可以看到四名可扮演角色之一的亞洲女性 Xian Mei 的背影，然後她將會遇到倖存的救生員 Sinamoi，並且接受相關任務。在執行任務過程中，將面對成群殭屍的攻擊，還會出現被拘束具束縛的殭屍等不同模樣的殭屍來襲；從圖片中，可以看到玩家可以使用包括球棒、手槍等方式，來保護自己。
